This form is designed for students who are already enrolled in an undergraduate degree (not CCP/dual enrollment college courses). If you are interested in learning more about the undergraduate degree program that will best prepare you for graduate school, please complete the Undergraduate Request for Information Form, visit the first-year admissions webpage or reach out to cahs.admissions@uc.edu for more information. Our admission counselors are better equipped to guide you through the admissions process at an undergraduate level.
